Requirements
- High School Diploma, BA preferred
- 3-5 years' experience in working with the homeless and exposure to full-spectrum Case Management.
Responsibilities
- Screens individuals for eligibility in the Rapid Re-housing Program.
- Determines support services needed and establishes goals with participant to aid in achieving self-sufficiency.
- Establishes relationships with community organizations and providers through community outreach and follow-up.
- Establishes and maintains protocols with referents throughout community. Has a thorough understanding of and ability to implement Federal grant processes under guidelines of OASAS and OMH.
- Maintains documentation according to OASAS, JCAHO, DOH, HUD, and any other designated governing body.
- Performs other duties as assigned by the Community Support Services Program Manager or the Executive Director.
- Maintains documentation through word processing and personal computer skills, when applicable.
