About The Position
Milford Public Schools is seeking a full-time Special Education Transition Teacher for the 18–22 Year Old Transition Program. This position is available ASAP. Milford Public Schools is seeking a certified Special Education Teacher to support students ages 18–22 in our specialized Transition Program, including students with medically fragile needs. The program focuses on developing independence, functional life skills, and meaningful post-secondary outcomes through building-based and community-based vocational experiences.
Requirements
- Current CT Special Education certification (165 or 065) required.
- Strong collaboration, communication, organization, and problem-solving skills.
- Commitment to supporting students in developing independence, self-advocacy, and meaningful post-secondary outcomes.
Nice To Haves
- Experience working with students with complex medical, developmental, or intensive support needs preferred.
- Knowledge of transition planning, vocational instruction, and functional life skills programming preferred.
Responsibilities
- Develop and implement individualized instruction aligned with students’ IEP goals, emphasizing functional academics, independence, communication, and vocational skills.
- Provide instruction and support to students with complex learning, medical, and developmental needs in a safe, inclusive, and student-centered environment.
- Facilitate building-based and community-based vocational opportunities that promote employment readiness, independence, and successful transition into adulthood.
- Collaborate with families, related service providers, paraprofessionals, medical professionals, and community partners to ensure comprehensive student support.
- Develop transition plans and instruction aligned with students’ post-secondary goals and individual needs.
- Implement evidence-based instructional strategies, positive behavioral supports, and appropriate accommodations.
- Maintain accurate IEP documentation and ensure compliance with IDEA, Connecticut special education regulations, and district procedures.
