Supports individuals diagnosed with mental illness, developmental and/or physical disabilities, and other medical co-existing conditions to have access to community resources, based on assessment of participant needs. Conducts home visits to perform evaluations and track progress of participant. Develops an individual service plan (ISP) and a community outreach support network strategy; supports participant and family/caregiver to access/apply for services, and provides ongoing support to help the individual achieve desired outcomes.
