About The Position

Trillium Health Partners is one of Canada’s largest community-based teaching hospitals, serving the growing and diverse populations of Mississauga, West Toronto, and surrounding communities. This position is within the Mental Health Program, specifically the Early Psychosis Intervention Program (NAVIGATE). The role involves providing clinical assessment and evidence-based treatment for adolescents and young adults experiencing their first episode of psychosis. The Social Worker will collaborate with clients, families, and the treatment team to develop, implement, and evaluate treatment goals, provide education and support to families and significant others, consult with community agencies for treatment coordination, and perform case management duties.

Requirements

  • M.S.W. from an accredited Faculty of Social Work.
  • Member of the Ontario College of Social Workers and Social Service workers (OCSWSSW).
  • Registered Social Worker (RSW).
  • Minimum three years post graduate experience working with adolescents and young adults experiencing first episode psychosis.
  • Experience working individually and in groups.
  • Experience in providing wide range of treatment services and approaches, including cognitive behavioural therapy, and psycho-educational groups for adolescents and young adults.
  • Thorough knowledge of adolescent development, family dynamics and systems.
  • Excellent interpersonal, communication and organizational skills combined with the ability to work independently as well as part of an inter-professional team.
  • Excellent assessment and documentation skills.
  • Positive work and attendance record.

  • Knowledge of the NAVIGATE model an asset.

Responsibilities

  • Provide clinical assessment and evidence-based treatment for adolescents and young adults with first episode psychosis (NAVIGATE).
  • In partnership with clients and families/significant others and treatment team, develop, implement and evaluate overall treatment goals.
  • Educate and support families/significant others.
  • Consult with community agencies to maintain coordination in the client’s treatment process.
  • Case management.
