About The Position

Capstone Hospice is seeking a Social Worker to join their dedicated hospice team, covering the North Gwinnett areas of Suwanee/Lawrenceville. This full-time position involves providing a wide range of psychosocial services to terminally ill/chronically ill patients and their families. Services include assessing psychosocial and pre-bereavement needs, coordinating care, and providing support in various settings such as residential care, assisted living facilities, personal care homes, and private residences. The role emphasizes making a meaningful impact on patients and families during a crucial time, with a mission to bring peace and comfort. Requirements

  • Licensed degree in social work; LMSW or LCSW.
  • Minimum 2 years of experience as a social worker.
  • Acceptance of and adaptability to diverse family environments, different socio-economic, cultural, racial and religious modes.
  • Must have current valid driver's license, auto liability insurance and reliable transportation.
  • Must provide evidence of annual TB test, COVID vaccination at time of hire.
  • Ability to work independently.
  • Ability to make accurate, and at times quick judgements.
  • Ability to respond to crisis outside of an office/hospital setting.
  • Ability to work within an interdisciplinary setting as part of a team.
  • Ability to be flexible, innovative and possess good interpersonal skills.
  • Ability to cope with mental and emotional stress and demonstrate emotional stability.

Responsibilities

  • Provide a wide range of psychosocial services to terminally ill/chronically ill patients and their families.
  • Regularly assess the patient and family's psychosocial needs and pre-bereavement/anticipatory grief needs.
  • Coordinate care for patients and families.
  • Provide education, outreach, and referral as needed.
  • Create and implement care plans to meet patient and family needs, including understanding Hospice/Palliative care, completing Advanced Directives, funeral/cremation planning, facility collaboration, level of care changes, environmental safety/fall risk management, family dynamics/caregiving needs, signs and symptoms of death/decline, identifying abuse/neglect potential, psychosocial well-being (anxiety/depression), improving quality of life and decreasing stressors, care planning for suicide risk/ideation, and assistance with financial resources and Veteran benefits.
  • Adhere to company policies and procedures, regulatory compliance requirements, and directives.
  • Perform initial psychosocial and pre-bereavement assessments and assist in the development and implementation of the goal-directed interdisciplinary plan of care.
  • Provide ongoing assessment and updates to care plans to address ongoing needs.
  • Provide short-term crisis intervention and individual or family counseling when indicated.
  • Observe, assess, and bring to IDG conferences information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family.
  • Act as an advocate for the patient/family unit.
  • Develop and maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate referrals.
  • Document comprehensive psychosocial assessments clearly, concisely, and in a timely manner.
  • Accurately document visits, phone contacts, and referral actions per agency policy.
  • Actively participate in staff meetings, department meetings, team meetings, case conferences, in-services, and other assigned meetings.
  • Complete all orientation requirements.
  • Participate in orienting new staff as needed.
  • Assume responsibility for personal and professional development of skills.
  • Maintain all appropriate state licensing requirements.
  • Assure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
