About The Position

As a Social Worker, you'll be at the forefront of building meaningful relationships with our residents, their families, and designated representatives. You'll be their go-to person during important transitions between different levels of residency or other life changes, offering guidance, support, and a friendly ear to listen. Your role as a valued member of our interdisciplinary team will also involve contributing to Person-Centered Care Planning and, with our Director of Social Work, ensure we meet all regulatory requirements for our 48-bed skilled nursing department within our vibrant life plan community. You’ll assist with quality assurance initiatives, facilitate various resident support groups and staff education as needed and act as a liaison to the Long-Term Care Ombudsman program.

Requirements

Masters or Bachelors degree in social work
2 years of experience
Experience in long-term care, health care, end-of-life care, or gerontology
Excellent interpersonal and time management skills
Ability to collaborate effectively within an interdisciplinary team
Proficiency in computer programs such as Word, Excel, Outlook, etc.
Familiarity with Electronic Medical Record Software
