About The Position

Within Southern NH Medical Centers Behavioral Health Unit, we understand the important connection between physical, mental, and emotional well-being and have the resources to support our community through inpatient and outpatient behavioral health services, from emergency care, to intensive outpatient programs. This 18-bed voluntary unit serves as an essential healing process within our community.

Requirements

• Master's Degree in Social Work required
• Minimum of two years acute care social work experience preferred.

Nice To Haves

• Professional certification in social work
• LICSW licensure or additional certifications are desirable but not required.

Responsibilities

• Conduct patient assessments, utilization reviews
• Provide resources and community referrals for patient support services
• Participate in discharge rounds and facilitate case conferences
• Care coordination with families, providers, and the clinical team
• Facilitate staff de-briefing meetings
