About The Position

This Social Worker position is part of the Clinical Case Management team at Atrium Health Pineville. The role involves completing psychosocial assessments, assisting patients with coping mechanisms related to their illness, and collaborating with the Clinical Care Management team to determine appropriate post-hospital care settings. The Social Worker will also serve as a resource for hospital staff and physicians regarding the psychosocial aspects of patient care, provide information on community resources, make referrals, and act as a liaison with the Department of Social Services. The position requires availability for occasional weekend coverage and responding to consultative requests after normal working hours.

Requirements

  • Masters in Social Work required
  • Applicable state Certification is preferred.
  • Adherence to National Association of Social Workers Code of Ethics.
  • Expertise with Data Management Tools.

Nice To Haves

  • LCSW preferred
  • At least 1 year professional experience in hospital or health related setting preferred.

Responsibilities

  • Completes psychosocial assessments for patients' post-hospital care and their support systems.
  • Assists patients in coping with stress related to hospitalization, disability, and chronic/terminal illness.
  • Works with the Clinical Care Management team to assess and evaluate the patient for the appropriate level of care or environment setting.
  • Serves as a resource to hospital staff and physicians regarding emotional, social, and psychosocial components of the patient's illness.
  • Provides information and education to patients on community resources and options for post-hospital care.
  • Makes referrals to community agencies as needed.
  • Reports suspected cases of child & adult abuse/neglect/exploitation.
  • Serves as liaison between hospital, patients, and Department of Social Services during evaluation/investigation.
  • Responds to requests for consultative services to patients after normal working hours.
