About The Position

This is a full-time, day shift position (8:00 am - 4:30 pm, Monday-Friday) focused on case management. The role involves discharge planning in an acute care or ambulatory setting, requiring strong clinical assessment, communication, and interpersonal skills. The social worker will collaborate with physicians, payers, colleagues, patients, and families, demonstrating sound judgment and a professional demeanor. Knowledge of utilization management, care coordination, transitions of care, and community resources is essential.

Requirements

• Bachelor degree in Social Work
• Current LSW license to practice as a social worker in the State of Ohio
• Minimum 1 year experience in discharge planning in an acute care setting or an ambulatory setting
• Effective Communication Skills
• Strong organization skills
• Self-directed
• Flexible
• Able to adapt to rapidly changing regulatory requirements
• Excellent clinical assessment and analytical skills
• Computer competency
