About The Position

Provides skilled interventions for the support and/or resolution of patient and family crises, problem-solving and decision-making, advocacy and facilitation necessitated by life-changing events (adoption, guardianship, abuse, placement, bereavement), mediation of risk factors and coping with disease or disability, and other psychosocial and/or socioeconomic issues while adhering to Saint Francis Health System and departmental policies/procedures. This role involves comprehensive case management, discharge planning, continuing care services, advocacy, clinical social work services, crisis intervention, and patient/family education. The social worker will also address issues related to abuse and neglect, advanced directives, psychiatric placements, and end-of-life care. Collaboration with interdisciplinary teams and utilization of hospital and community resources are key aspects of this position.

Requirements

  • Completion of Masters of Social Work or closely related degree from an accredited program.
  • Must be computer literate.

Nice To Haves

  • Experience in a hospital or community health agency is preferred.
  • Prior experience with assigned patient population may be required.
  • Additional language fluency in addition to English may be preferred.
  • LCSW (Licensed Clinical Social Worker) or LPC (Licensed Practical Counselor) is preferred.

  • Collaborates with interdisciplinary teams for discharge and transitional planning.
  • Participates in community education and support group activities.
