We are currently seeking a Part Time Social Worker (SNF) to coordinate or perform the necessary administrative functions regarding the admissions/discharges of residents in healthcare. Requirements

  • Ability to read, write, speak and understand English fluently.
  • Ability to meet or exceed the company’s attendance and punctuality standards.
  • Ability to use Electronic Records and miscellaneous software and office equipment.
  • Ability to understand and follow directions as given.
  • Ability to work with minimal supervision.

Nice To Haves

  • Bachelor’s degree in Social Work, or closely related human services field desired.
  • 1-2 years related experience in a Healthcare setting.
  • Two years’ experience in a similar social work position desired.
  • Two years of long-term care or rehabilitation experience desired.

Responsibilities

  • Assists with the responsibilities and functions delineated in the discharge plan.
  • Ensures that the administrative and clerical functions related to the admission and/or discharge of residents is carried out to completion by coordinating efforts with the Healthcare Team Members.
  • Maintains appropriate contact with hospitals, doctors and outside agencies to facilitate admissions.
  • Coordinates all forms needed for admissions.
  • Maintains state required resident roster.
  • Coordinates financial updates for private pay residents.
  • Completes appropriate section of the MDS.
  • Participate and attend all in-service training and education programs as scheduled.
  • Other duties as assigned.
