Requirements

  • Bachelor degree in Social Work, or equivalent, and Registered Social Worker designation in Ontario
  • Minimum 2 years experience in counseling or group work, preferably in a long term care environment
  • Proficiency in computer applications
  • Ability to work independently, be self-directed to problem solve, exercising good judgment, tact and diplomacy
  • Empathetic attitude and desire to contribute towards the wellbeing of the elderly
  • Excellent communication and interpersonal skills
  • Satisfactory Police Vulnerable Sector Screen result
  • Ability to converse in Chinese.

Responsibilities

  • Using professional case management practice, carries out intake assessment, service coordination, referrals, case conferencing and case monitoring
  • Provides supportive counselling to clients and their family caregivers on an individual and group basis
  • Coordinates with interdisciplinary team members to carry out and monitor resident care plan pertaining to social work support
  • Offers instrumental help to assist the client to resolve functional problems of daily living
  • Provides crisis intervention and short term assistance in addition to maintaining a caseload assignment
  • In consultation with the Executive Director, develops and implements policies pertaining to social work services
  • Participates in and contributes to interdisciplinary team conference of the LTC Home
  • Works closely with the Executive Director in providing social work services to residents of the LTC Home
  • Actively participates and contributes to the overall LTC leadership team meetings and functions
  • Keeps up-to-date with developments and best practices in the field
  • Other duties as assigned
