About The Position

DaVita is hiring a Social Worker to serve as a vital member of each patient’s core care team in an outpatient dialysis setting. The role supports and advocates for patients as they adjust to life on dialysis and manage the physical, mental, emotional, and financial demands of End Stage Renal Disease. The Social Worker helps patients understand their rights and responsibilities and builds meaningful, long-term relationships with patients and their families. The position is part of a supportive team environment that values work-life balance, personal and professional growth, and patient-centered care. The role also involves contributing to patient health through clinical goal-setting and quality improvement initiatives.

Requirements

Master's Degree in Social Work (MSW) with a specialization in clinical practice from an accredited school of Social Work.
Meet all state-required regulations to practice in the dialysis setting.
Hold a clinical license (e.g., LCSW, LICSW) and meet all state licensure requirements.
Proficiency in all Experienced-level Social Worker duties.
Intermediate computer skills.
Demonstrated knowledge of government and private insurance programs.
Excellent interpersonal and communication skills.
