About The Position
The Social Worker is a vital member of each patient’s core care team, supporting and advocating for patients as they adjust to life on dialysis. The role helps patients understand their rights and responsibilities and guides them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. The position involves building meaningful, long-term relationships with patients and their families in an outpatient setting and working as part of a supportive team.
Requirements
- Meet all state-required regulations to practice in the dialysis setting.
- Master's degree in Social Work (MSW) with a specialization in clinical practice.
- Demonstrated knowledge of government and private insurance programs.
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
- Functional proficiency with DaVita-specific applications within 60 days.
Responsibilities
- Support and advocate for patients transitioning to life on dialysis.
- Help patients understand their rights and responsibilities.
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
- Build meaningful, long-term relationships with patients and their families.
