About The Position

Family Service League is seeking a full-time, Master’s-level Social Worker for our School Based Program in Riverhead, NY. The Social Worker will provide individual, family and group therapy for adolescents with a full range of DSM diagnoses in an educational setting, connected to outpatient mental health clinic.

Requirements

• Master’s degree in relevant human service field required.
• LMSW, LMHC, LCSW or related licensed clinical degree or limited permit required.
• At least one year of experience working in a behavioral health setting required.
• Proficient computer skills, including Microsoft Office and Electronic Health Records required.
• Excellent interpersonal, and verbal and written communication skills required.
• Must possess the ability to multitask, problem solve, make clear decisions, and work individually and as part of a team.

Nice To Haves

• Experience providing school-based mental health counseling is strongly preferred.
• Bilingual in Spanish preferred, but not required.

Responsibilities

• Provide individual and group therapy to children/ adolescents with full range of DSM diagnoses in a school-based setting. Direct service will be provided both in person and via telehealth
• Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record, including progress notes, comprehensive assessments and Individual Actions Plans.
• Maintain ongoing communication and collaboration with school personnel, parents/guardians, and community providers.
• Provide collateral contacts with all relevant referral sources, including family members.
• Coordinate linkage to community resources and discharge planning.
• Attend routine supervision/team meetings.
• Understand and demonstrate a commitment to Performance Quality Insurance (PQI) and accreditation with the Council on Accreditation for Children and Family Services (COA).
• Provide additional support/services to the clinic as needed.
• When school is not in session, or as needed during the year, provide services in the outpatient clinic.
• All other duties as assigned.
