MSW / Social Worker - Hospice
About The Position
Haven Hospice is seeking a Social Worker in DeLand, Florida, covering DeLand and the surrounding area. The Social Worker provides emotional support, counseling, and resource coordination to patients facing terminal illness, helping ensure their psychosocial needs are addressed with dignity and respect. The role contributes to holistic, person-centered hospice care and involves working independently with support from the care team. The position is based at 929 North Spring Garden Avenue, Suite 135, DeLand, FL 32720, with a Monday–Friday schedule from 8 a.m. to 5 p.m. The role supports patients and families at home and provides opportunities to build skills in grief counseling and long-term emotional support. This position is subject to a Level 2 background screening.
Requirements
- Graduate of a Council on Social Work Education (CSWE)-accredited master's program in social work (MSW) or bachelor's program in social work.
- Meet personnel qualifications stated in §418.114(b)(3).
- At least one year of experience in health care.
- Understand hospice philosophy and issues related to death and dying.
- Demonstrate good verbal and written communication and organizational skills.
- Hold current licensure, according to Medicare guidelines, to practice in the state of operation.
- Be a licensed driver and have an automobile that is insured in accordance with state or organization requirements and in good working order.

Responsibilities
- Assess the psychosocial needs of patients and families.
- Evaluate home safety and assist with alternative care plans when needed.
- Provide counseling, crisis intervention, and emotional support.
- Maintain accurate and timely clinical documentation.
- Educate patients and families about advance directives and assist with them.
- Provide referrals and practical support for patients and families.
- Serve as a liaison with community resources and agencies.
- Collaborate with team members to support holistic patient care.
- Support funeral, financial, and legal planning with families.
