About The Position

Providence Care is hiring a Licensed Masters Level Social Worker to join their growing team. This full-time position involves providing LMSW services to hospice patients in their homes, assisted living facilities, or skilled nursing facilities within Rock Hill, SC, and surrounding counties. The Hospice Social Worker is responsible for the provision, direction, supervision, and evaluation of medical social services to clients in the home environment, ensuring compliance with agency policies, health standards, and governmental laws and regulations. The role focuses on proactively meeting client needs to achieve individualized and measurable outcomes.

Requirements

• Graduate degree in social work or related field
• LMSW or independently licensed LISW-CP in South Carolina
• Excellent organizational, communication, and time management skills.
• Minimum of two years social work experience in a healthcare setting.
• Excellent interpersonal and analytical skills.
• Willingness to travel across several counties

Nice To Haves

• Independently licensed LISW-CP in South Carolina with the ability to provide clinical supervision.

Responsibilities

• Participates in development and re-evaluation of POC for clients needing MSW.
• Instructs and counsels the client and family in treating and coping with the social and emotional responses to illness.
• Participates in discharge planning.
• Participates in care coordination activities and act as resource to other team members in identification and resolution of client needs.
• Utilizes and connects patients and their families with community resources.
