About The Position

We are seeking a Social Worker 3 or 4 to provide direct clinical services to individuals, families, and groups using appropriate therapeutic modalities. Requirements
- Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW)
- A master's degree in social work
- Two years of social services experience
Responsibilities
- Provides direct clinical services to individuals, families, and groups using appropriate therapeutic modalities.
- Offers case consultation to case managers and other agency providers regarding individual clients.
- Conducts risk assessments and psychosocial evaluations on crisis cases; assists psychiatrists/medical providers in determining appropriate treatment or disposition.
- Maintains compliance with federal and state regulatory standards, as well as CLHSD policies and procedures.
- Participates in the Quality Assurance (QA) process to ensure all standards are met.
- Assists in the implementation and ensures ongoing compliance with CARF standards.
- Administers Child and Adolescent Functional Analysis Screen for Flexible Family Fund Program. (ACT 378)
- Develops Safety Plans on Adult Behavioral Health clients as needed.
- Coordinates and manages orientation and aftercare for clients recently discharged from inpatient psychiatric hospitalization and referred to the clinic for services.
- Completes required initial paperwork for new clients.
- Prepares discharge summaries and completes chart closures in the electronic health record system.
- Participates in clinic-wide or regional committees, planning, and management teams as directed by the Adult Services.
