The Clinic Social Worker will perform culturally responsive psychosocial screening and assessment to identify social, psychological, behavioral, economic, and environmental needs of outpatients and their families. This role involves developing goals related to these needs, identifying potential risks and barriers to positive health outcomes, and providing social work and therapeutic interventions. The social worker will also provide education on Advance Care Planning, facilitate transitions to home and community, and assist with problem-solving barriers to care. This position is an integral member of the health care team, supporting a team-based approach to care management and patient navigation. The role also includes participation in research activities and adherence to evidence-based practices and ethical standards.
