About The Position

NHC is seeking a full-time Social Worker to advocate for patients and their families in a healthcare setting. The qualified applicant should have a bachelor's or master's degree in social work from an accredited program and knowledge of case management and the needs of geriatric patients, including funding resources. The position offers a team-oriented environment and opportunities for growth.

Requirements

Bachelor's or master's degree in social work from an accredited program.
Knowledge of the medical case management process and the needs of geriatric patients.
Working knowledge of healthcare settings and funding resources, including Medicare Parts A, B, C, and D, Medicaid, managed care, and other private insurance.
Imagination and creative ability; skill in working cooperatively with other professionals; and the ability to organize and carry out responsibilities efficiently and effectively.

Nice To Haves

Bachelor's degree from an accredited college or university preferred.
Insurance authorization experience preferred.
Medical knowledge and terminology preferred.
Marketing experience preferred.

Responsibilities

Work with individual patients and families to support adjustment to center life and to crises involving illness, disability, and death.
Advocate for patients and families as they express, define, and resolve grievances.
Coordinate discharge planning functions.
Report the current status and present needs of social services to the Director of Social Work Services.
Participate in developing patient care plans in cooperation with professionals from other disciplines.
Maintain confidentiality of professional information.
