About The Position

Adoration Home Health is seeking a dedicated Home Health Social Worker to provide patient-centered care in Cullman, Alabama, serving Cullman, Blount, and Marshall counties. The role involves providing one-to-one visits and supporting patients and families with social, emotional, financial, and health-related needs. The position is scheduled PRN and offers flexible scheduling, independent work with team support, and opportunities for advancement.

Requirements

• Master’s degree in Social Work.
• Five or more years of clinical social work experience.
• Valid, current Licensed Clinical Social Worker or Licensed Medical Social Worker license.
• Ability to conduct psychosocial assessments and develop and implement viable care plans.

Responsibilities

• Guide and counsel individuals and families experiencing crises, including people who are unemployed, children needing foster homes, and elderly people without caregivers.
• Assist clients in obtaining government funds, education, or treatment.
• Gather and evaluate medical, psychological, and social information relevant to clients’ situations, along with their capacities.
• Discuss clients’ concerns—including personal and family adjustments, finances, employment, food, clothing, housing, and physical or mental impairments—to determine the nature and degree of their problems.
• Help patients and families plan how to meet their needs through individual or group conferences, and help clients mobilize personal capacities and environmental resources to improve social functioning.
• Help clients modify attitudes and behavior patterns by increasing their understanding of themselves, their problems, and their role in creating them.
• Develop a service plan intended to restore patients to the best social and health adjustment possible within their capacity.
