About The Position

The Social Worker at German Centre Home provides compassionate, comprehensive support to residents and their families. The role conducts psychosocial assessments, develops individualized care plans, provides counseling and interventions, advocates for resident rights and quality of life, and coordinates with an interdisciplinary care team. The Social Worker also connects residents and families with community resources and post-discharge services to support successful care transitions. The position is full-time, Monday through Friday, with no weekends or holidays.

Requirements

• Bachelor or Master of Social Work (BSW or MSW).
• Current, unencumbered Massachusetts LSW, LCSW, or LICSW license.
• Ability to communicate fluently in English, verbally and in writing.
• Must be able to provide a recent physical and required immunizations.
• Candidates are subject to extensive background checks and professional reference checks.

Nice To Haves

• Prior experience in a long-term care setting is strongly preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized care plans addressing residents’ emotional, social, and discharge planning needs.
• Provide counseling, intervention, and ongoing support to residents and their families.
• Advocate for resident rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services and facilitate care conferences.
• Help ensure resident-centered care.
• Connect residents and families with community resources and post-discharge support services to promote successful care transitions.
