About The Position

Social Work Services at the Mount Sinai Hospital The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities. The social worker in this position will facilitate the discharge of patients in the ED.

Requirements

NYS LMSW or LCSW.
Strong interest in being part of innovative initiatives to improve the quality of care for patients who present in ED/RETU.
Excellent clinical skills, with exceptional skills in rapid psychosocial assessment, crisis intervention, and focused short-term treatment.
Ability to prioritize effectively multiple, simultaneous requests for service and respond effectively to the urgent needs of a diverse patient population.
Strong interdisciplinary/collaborative skills and organizational skills.
Exceptional skills in engaging patients and families and expertise in coordinating resources within the hospital and community networks for effective management of patient care.
Internal candidates: Minimum one year in current position unless in the ED/ RETU.

Responsibilities

Facilitate the discharge of patients in the ED.
Perform psychosocial assessment.
Provide a full range of psychosocial interventions for patients/families.
Manage transitional care.
Conduct follow-up phone calls when appropriate.
Provide post-discharge intervention.
Offer counseling.
Provide crisis intervention (including cases of domestic violence, child abuse and neglect, sexual assault, human trafficking, and death).
Assist with concrete services (e.g., pharmacy, home care, transportation, entitlements, referral for community services).
Facilitate placement of patients in facilities.
Collaborate closely with social workers in outpatient settings and Care Coordination programs.
Provide coverage for urgent needs that arise after hours across the hospital.
