About The Position
Social Work Services at the Mount Sinai Hospital is seeking a Social Worker to facilitate the discharge of patients in the ED. The Mount Sinai Hospital is a large facility with an extensive outpatient and specialty care network, offering comprehensive social work services to diverse populations. The department emphasizes a holistic approach to patient care, addressing physical, emotional, and social needs. Social Workers collaborate within interprofessional teams, engaging in both direct care and prevention at micro and macro levels. The Department of Social Work Services, founded in 1907, is one of the oldest hospital social work departments in the nation, employing over 450 licensed social workers across more than 65 program/service areas. Requirements
- NYS LMSW or LCSW.
- Strong interest in being part of innovative initiatives to improve the quality of care for patients who present in ED/RETU.
- Excellent clinical skills, with exceptional skills in rapid psychosocial assessment, crisis intervention, and focused short-term treatment.
- Ability to prioritize effectively multiple, simultaneous requests for service and respond effectively to the urgent needs of a diverse patient population.
- Strong interdisciplinary/collaborative skills and organizational skills.
- Exceptional skills in engaging patients and families.
- Expertise in coordinating resources within the hospital and community networks for effective management of patient care.
- Internal candidates: Minimum one year in current position unless in the ED/ RETU.

Responsibilities
- Facilitate the discharge of patients in the ED.
- Perform psychosocial assessments.
- Provide a full range of psychosocial interventions for patients/families.
- Manage transitional care.
- Conduct follow-up phone calls when appropriate.
- Provide post-discharge intervention.
- Offer counseling and crisis intervention, including cases of domestic violence, child abuse and neglect, sexual assault, human trafficking, and death.
- Assist with concrete services such as pharmacy, home care, transportation, and entitlements.
- Refer patients for community services.
- Facilitate the placement of patients in facilities.
- Collaborate closely with social workers in outpatient settings and Care Coordination programs.
- Provide coverage for urgent needs that arise after hours across the hospital.
- Work THREE 12.5-hour days weekly for three weeks/month and FOUR 12.5 hour days one week/month.
- Work weekend days on a rotating schedule.
- Work overnight hours, 7 PM – 7:30 AM.
- Adjust hours during orientation and training period for daytime work.
- Adjust hours for clinical supervision as needed.
