Social Worker - Ambulatory Care Network - Days Our Broadway Practice which is part of our Ambulatory Care Network provides services such as triage, screening, referral, and crisis intervention to ensure the mental well-being of our patients. You will be responsible for seeing patients virtually and in person, via scheduled appointments and walk ins. Position involves a wide range of responsibilities including but not limited to: crisis intervention, assessing for safety using evidence based screening, concrete services, assisting and following up with home care, psychosocial assessments, referrals to CBO's. Must work collaboratively with interdisciplinary team and across the network of ambulatory care clinics. This is a Monday - Friday 1199 union position. This is a 9:00 AM - 5:00 PM except Thursdays 9:30 AM - 5:30 PM.

Requirements

Master's Degree in Social Work (MSW)
New York State License as a Licensed Masters Social Worker (LMSW)

Nice To Haves

Bilingual Spanish
Experience in a busy outpatient setting
Experience working with interdisciplinary teams
