The Ambulatory Care Network's Broadway Practice provides services such as triage, screening, referral, and crisis intervention to ensure patient mental well-being. The role involves seeing patients virtually and in person, handling scheduled appointments and walk-ins. Responsibilities include crisis intervention, safety assessments using evidence-based screening, concrete services, home care assistance and follow-up, psychosocial assessments, and referrals to Community-Based Organizations (CBOs). The position requires collaboration with an interdisciplinary team and across the network of ambulatory care clinics. This is a Monday-Friday, 1199 union position with specific hours: 9:00 AM - 5:00 PM, except Thursdays 9:30 AM - 5:30 PM. All roles require on-site presence, and employees must live within a commutable distance to NYP. NYP will not reimburse for travel expenses.
