About The Position

One full-time position available on the main Livestock Exchange Campus (30th & L), operating Monday - Friday from 8:00 am to 5:00 pm. The Social Worker performs a variety of social work services, from simple to crisis cases, assessing patient and family needs, developing appropriate programs, investigating service needs, implementing treatment plans, preparing records, and performing other related duties. This role requires a working knowledge of social work principles applied to individual and family problems. The Social Worker is a member of an integrated health care team, utilizing interpersonal skills for complex interactions, serving as a resource for clinical staff on psychosocial issues and community resources, and creating resources for the team. In patient care, the Social Worker applies social work principles to assess, develop, and provide direct services, helping individuals and families cope with problems, understand their social situations, and access community resources. They assist with medication, transportation, housing, and other social determinants of health, acting as a point of contact for abuse or neglect cases, and conducting screenings and education. As a patient advocate, the Social Worker champions patient rights and organizational ethics, with a commitment to diversity, equity, inclusion, community engagement, and social justice. In case management, they identify and assess services for social, financial, and emotional needs affecting medical conditions, treatment, recovery, and transitions, maintaining a file of community resources and assisting patients with referrals and paperwork. For outreach services, the Social Worker acts as a liaison between OneWorld and community resources, networking with external organizations for collaborations and partnerships, and conducting presentations. The role also involves providing orientation, training, and supervision to social/human services practicum students. The outlined duties are a general overview and not exhaustive; additional tasks may be assigned.

Requirements

  • Bachelor’s Degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
  • Certified Social Worker (CSW) by Nebraska State Law is preferred upon hiring. If not certified at the time of hire, certification will be required within 90 days of employment.
  • Excellent verbal and written communication skills; English/Spanish proficiency required.
  • Task-oriented and detail-oriented with excellent customer service and organizational skills.
  • Great computer proficiency with Microsoft Office and a willingness to learn other programs.
  • Working knowledge of the Latino culture and commitment to cultural competence.
  • Familiarity with social service resources in the community.
  • Willingness to complete home visits as deemed necessary (Motor Vehicle Report (MVR) required) and a valid driver’s license.
  • Functions well in a team environment.
  • Motivated and self-directed.

Nice To Haves

  • Prior experience in completing needs assessments and evaluations is preferred.
  • Two years social work experience in a healthcare or social services environment is preferred.
  • Experience working with Domestic Violence, Child Protective Services (CPS) and Adult Protective Services (APS) is a plus.

Responsibilities

  • Applies the principles, practices, and codes of ethics of social work services in the performance of assignments such as assessing, developing, and providing direct services or programs to individuals, families, and/or groups.
  • Assists individuals and/or families to find satisfactory ways of coping with and understanding the nature and extent of their problems in their social situations and relationships.
  • Assures that patients and families are knowledgeable of available community resources and how to access them.
  • Provides assistance with medication, transportation, housing, and other social determinants of health.
  • Acts as a point of contact for patients who present with any type of abuse or neglect.
  • Conducts screenings and education for patients related to their health and well-being.
  • Functions as an advocate for patient rights and organizational ethics.
  • Identifies and assesses available services to meet the social, financial, and emotional needs that affect a patient's medical condition, treatment, recovery, and transition to a lower level of care.
  • Maintains an ongoing current file of community resources and programs.
  • Assists patients with formal and informal referrals, phone calls, and completion of paperwork and applications.
  • Acts as a liaison between OneWorld and community resources.
  • Networks with external organizations and groups with the goal of creating collaborations and partnerships that benefit OneWorld patients.
  • Conducts internal and external presentations as needed.
  • Provides orientation, training, and supervision to social/human services practicum students.
