About The Position

Bayshore HealthCare is seeking passionate and caring Social Workers for its Integrated Care Solutions team. The role involves applying social work principles in a community setting to assist adult clients in maximizing their social, emotional, and cognitive functioning, thereby promoting independence and improving quality of life. The position emphasizes Person-and Family-Centered Care (PFCC), which involves active client and family participation in healthcare decisions. This is a part-time, home care position serving the North Simcoe Muskoka region (primarily Collingwood/Midland), requiring 2-3 days of daytime availability Monday to Friday, with approximately 15 visits per week. Compensation includes a per-visit rate, mileage between clients, and a separate virtual visit rate.

Requirements

  • Be an active member in good standing of the Ontario College of Certified Social Workers and Social Service Workers.
  • Possess general computer skills and have access to a computer with appropriate password protection and internet capabilities.
  • Excellent command of the English language, both written and oral.
  • Provide a police clearance (Vulnerable Sector Check) that meets the Bayshore Home Health policy.
  • Hold a current CPR certificate (CPR-C or BLS and is only acceptable a year after issue).
  • Possess strong work ethic with commitment to regular attendance.

Nice To Haves

  • Minimum of two (2) years of community-based therapy experience.
  • French language skills.
  • Ability to drive, hold a current, valid driver’s license, and have access to a vehicle in good operating condition.

Responsibilities

  • Communicate with clients, families/caregivers, service providers, and other professionals to collaborate and coordinate services.
  • Maintain and manage a caseload efficiently, ensuring priority code timeframes are met.
  • Assess client adjustments to medical, emotional, psychological, or social problems and influencing factors.
  • Utilize a client-centered approach to identify, prioritize, and monitor potential psychosocial problems and goals.
  • Establish, implement, evaluate, and adjust the psychosocial treatment plan in collaboration with the client/family, caregivers, care coordinator, and community partners.
  • Enhance or teach coping skills such as relaxation, stress management, anger management, and healthy grieving.
  • Advise clients/caregivers on available community resources and contact agencies on their behalf when appropriate (e.g., Federal, Provincial, Municipal, Local agencies for income security, pensions, financial assistance).
  • Determine when further social work intervention is no longer necessary and assist clients in planning for discharge.
  • Recommend and facilitate referrals to community support services.
  • Develop a flexible, responsive system to meet changing and competing demands.
  • Demonstrate knowledge of Standards of Practice guidelines of the Provincial regulating body.
  • Liaise with other Case Managers, funders, or Government & hospital programs as required.
  • Liaise with the Provincial regulating body as required.
  • Complete other tasks as requested.
