This role involves working collaboratively with property management and other personnel to provide essential services to residents. The Social Worker will respond to referrals from property management, residents, and community members regarding challenges related to daily functioning. A key aspect of the role is client assessment, developing and overseeing case management plans, and emphasizing collaboration with community agencies. The Social Worker will assist residents with referrals to community resources, benefit programs, and other services, completing evaluations, plans, and service agreements when appropriate. Documentation of all contacts and activities is crucial, as is maintaining service records in the SHP database. The position also includes crisis intervention, counseling, case management, and referrals to long-term therapy services. Assisting with property activities related to Health and Community Services functions and performing other duties as assigned by the Clinical Supervisor and/or Director of Resident Services are also part of the role.
