About The Position

Under the oversight of a Licensed Clinical Social Worker (L.C.S.W.), the Social Worker provides psychological and social support to patients and families and may integrate counseling services with overall patient care. The role supports patients in meeting psychosocial needs through supervised mental health therapy practice or by facilitating adjustment to treatment, hospitalization, or transition into the community after therapy.

The position evaluates patients through interviews and psychosocial assessments to develop treatment plans and goals with the licensed treatment team. It provides approved counseling, social work assessments, and treatment to patients, families, or significant others; connects patients and families with community resources; and develops stress-management and wellness plans for inpatient and outpatient populations. Some case-management duties may also be performed.

• Ability to perform the essential functions of the job.
• Demonstrated human-relations skills and effective verbal and written communication skills.
• Ability to work in a multidisciplinary team, problem-solve, and resolve conflicts for individuals, couples, and groups.
• Ability to identify and arrange individualized community resources for patients, potentially including post-treatment financial assistance, shelter, and transportation.
• Ability to make recommendations based on experience regarding policy and service delivery.
• Knowledge and skills necessary to provide care appropriate to the ages of patients served on the assigned unit.
• Knowledge of life-span growth and development principles and ability to assess patient-status data and provide care in accordance with department policies and procedures.
• Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system; must be obtained within 30 days of hire.
• Either a current Utah Licensed Clinical Social Worker (L.C.S.W.) certificate for clinical practice or a current Utah Certified Social Worker license.
• Additional license requirements as determined by the hiring department.

Responsibilities

• Evaluate patients through interviews and psychosocial assessments to establish and develop treatment plans and goals in conjunction with the licensed treatment team.
• Administer approved counseling, including social work assessments, counseling, and/or treatment for patients, families, or significant others.
• Identify and provide outreach regarding community resources and referrals, including shelter, transportation, future treatment plans, and financial assistance.
• Develop and provide stress-management and wellness treatment plans for inpatient and outpatient populations.
• Perform some case-management duties as needed.
