Requirements
- Master’s degree in Social Work (MSW) from an accredited institution is required.
- At least one (1) year experience working with the frail or elderly population is highly desirable.
- Current valid Driver’s License, proof of auto liability coverage, and reliable transportation required.
Nice To Haves
- Current LCSW certification preferred.
Responsibilities
- Coordinate all transitions of care, mental, and psychosocial care.
- Counsel participants on financial and advanced care planning options including advanced directives, DNR, etc.
- Advocate for participants and ensure participants’ wishes are enacted.
