The purpose of this job is to provide rehabilitative programs and community resources to returning citizens housed in the Department of Corrections. Duties and responsibilities include, but are not limited to: working with care and treatment staff to implement unique rehabilitative plans for returning citizens to reduce recidivism; maintain a current network of resources to address housing, financial stability, mental wellness, education, substance counseling, and other varying needs as determined through assessments; maintaining case files; providing counseling and establish clear goals for each returning citizen; and develop and implement a schedule to track the success or recidivism rate of released participants for up to three years. To perform additional tasks as assigned.
