About The Position

The Complex Care Kidney Transplant Social Worker supports patients and families throughout the kidney and pancreas transplant journey. This role involves providing specialized psychosocial assessment, care coordination, and education within a fast-paced, highly collaborative transplant environment. The social worker will collaborate closely with transplant coordinators, financial teams, and a multidisciplinary care team to assist patients in preparing for transplant, navigating insurance and logistical challenges, and remaining engaged through evaluation, waitlist, and post-transplant phases. This position is ideal for social workers who enjoy complex medical work, value teamwork, and are adept at handling sensitive conversations with empathy and professionalism. Key aspects include recognizing psychosocial patterns, identifying potential barriers early, and proactively intervening to support transplant readiness and continuity of care. The role also involves educating patients and caregivers on transplant responsibilities, including caregiver expectations and required follow-through.
