About The Position

DaVita is looking for a Licensed Social Worker (LMSW) to support our patients and their families at our Moorhead (Dilworth, MN), and Fargo (Fargo, ND) Dialysis Clinics. Help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease. Requirements

Licensed or registered in the state of practice as required by state regulations -- LMSW or LCSW
Meets all state required regulations to practice in the dialysis setting.
Master's degree in Social Work (MSW) required with a specialization in clinical practice
Demonstrated knowledge of government and private insurance programs
Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days

Responsibilities

Support patients and their families at dialysis clinics.
Help patients understand their rights and responsibilities.
Guide patients in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.
Build meaningful and long-term relationships with patients and their families.
