About The Position

Project FIND's mission is to provide low- and moderate-income and homeless older adults with the services and support they need to enrich their lives and live independently. Today Project FIND operates three supportive housing residences that are home to about 600 people and four senior centers with over 3,000 members. Our members and residents range from the healthy and active to the frail and homebound to the homeless. For all these men and women, Project FIND is a critical resource, providing housing, meals, and programs that help individuals navigate the challenges of aging by encouraging community engagement and healthy living. The Social Worker will work with older adults aged 62 years old and older at the Park 79 location, which offers affordable housing administered by HPD through the Senior Affordable Rental Apartments Program. The program provides case management services, information and referral services, supportive counseling, housing assistance and maintenance services, group work, community building activities, and crisis intervention for clients and their families. The Social Worker is responsible for maintaining collaboration with the Property Management team, overseeing a caseload of clients, conducting detailed assessments to determine client needs and eligibility for benefits and services, and engaging with clients to develop service plans and address emergent needs.

Requirements

Master’s degree in social work or related field of study, or commensurate experience equaling a minimum of four years’ experience in the human services sector, required.
LMSW or LCSW strongly preferred with at least 2 years’ relevant experience in social service.
Experience working with older adult populations and families preferred.
Bilingual Chinese or Spanish language fluency preferred.
Must have strong computer skills including Microsoft Office.
Ability to provide accurate and timely documentation required.
Time management skills to coordinate multiple responsibilities and services.
Ability to work independently and as part of a team, in both a home and office environment.
Ability to maintain client and agency confidentiality.
Excellent interpersonal, public speaking, and written communication skills.
Must be able to communicate effectively and appropriately with other team members, community agencies, and clients with diverse opinions, values, characteristics, and cultures.

Responsibilities

Conduct intake and assessment screening, assessing mental health and physical health needs and support, and eligibility for benefits and entitlements; conduct home, office and community visits to address unmet needs.
Implement constructive short and long-term interventions through counseling, education about counseling and physical health supports, long-term planning, assistance with benefits and entitlements, and linkage and referral to community-based providers.
Provide orientation, support, and services necessary to assist tenants to remain permanently housed in a safe environment.
In collaboration with Property Management, assist with income affidavits, housing applications, recertifications and collecting rent as needed.
Interface with the Property Management team to resolve ongoing concerns or issues related to the residence, including both tenant and facility concerns and issues.
Develop and create training events to educate clients on topics that can help support them in their daily lives (e.g., self-care, health and wellness, legal planning).
Lead one or more support groups, providing emotional support and psychoeducation about various issues related to aging in place, health and wellness, and self-care.
Develop and create internal and external community building events and opportunities to facilitate activity and socialization that can help support clients’ ongoing wellness and quality of life.
Provide conflict resolution and engage in tenant concerns in matters requiring enhanced intervention.
Provide timely documentation and maintenance of client files and progress notes.
May provide supervision, training, and support to paraprofessional staff, including students.
Perform other related tasks as requested or assigned.
