About The Position

This is a travel assignment for a Social Worker in a behavioral health role, specifically within a surgical center setting in Tuba City, AZ. The contract is for 13 weeks, with a start date of 11/01/2026. The role involves outpatient/inpatient social work and requires 36 guaranteed hours per week on a 5x8 rotating shift. Shared contractor housing is available upon application.

Requirements

• Master’s degree in Social Work (MSW)
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW); valid, current, unrestricted license in any state
• Basic Life Support (BLS) via American Heart Association
• Minimum three (3) years in a clinical care setting
• Experience with discharge planning, case management, or utilization review
• Knowledge of electronic health record systems
• Ability to work in an inpatient/outpatient surgical center environment and participate in a travel assignment

Responsibilities

• Assess patients and develop care plans; coordinate a continuum of care to empower self-sufficiency
• Link patients to community, health care, substance use, and mental health resources; coordinate discharge planning and follow-up
• Review medical necessity for referrals to specialty services and collaborate with the care team to ensure appropriate resource use
• Communicate and coordinate with a multidisciplinary team to promote safe, efficient hospitalizations and smooth transitions
• Evaluate ED patients for care coordination needs and propose outpatient discharge plans with physician approval
• Manage care for patients with abuse, chronic pain, and frequent utilization, advocating for needed resources
