About The Position
Hunterdon Care Center is seeking a dedicated and compassionate CSW/LSW Social Worker to provide support, counseling, and case management services to individuals, families, and groups. The role focuses on supporting the senior population and providing high-quality care. The position is available part time or per diem at the Flemington, New Jersey facility.
Requirements
- Hold a valid New Jersey Social Work Certification (CSW/LSW) in good standing.
- Strong organizational skills and the ability to multitask effectively.
- Passion for working with the senior population.
- Excellent writing and documentation skills.
Nice To Haves
- Experience working in a nursing home facility is preferred.
Responsibilities
- Conduct admission and ongoing assessments, including documentation.
- Assist residents with adjustment to the facility.
- Complete and maintain MDS 3.0 documentation.
- Develop and participate in care planning.
- Assist with Medicaid applications and other financial matters.
- Advocate for residents’ needs and rights.
- Provide crisis intervention when necessary.
- Coordinate and support discharge planning.
