About The Position

The Case Manager provides social case management support as a
primary care provider to the client population. The population targeted for this
project are those who have difficulty engaging or have disengaged from care,
involved in substance abuse and who are impacted by substance use disorder and
Mental Health issues. "Preference for this position will be given to HSAS
members in accordance with the terms and conditions of the SAHO/HSAS Collective
Agreement"

Requirements

• 1 year experience working with marginalized population in a health-related field
• Bachelor of Social Work (BSW)
• Basic - Variety of computer programs
• Advanced - Mediation skills
• Advanced - Communication skills to transfer knowledge/skills
• Advanced - Problem solving skills
• Advanced - Communication skills
• Advanced - Assessment skills
• Knowledge in diverse cultures, values and traditions
• Ability to work as a member of a multi-disciplinary team
• Knowledge in the SHA, community resources and applicable programming
• Ability to be flexible and willing to change
• Ability to collect, record, report comprehensive client information accurately
• Ability to work and communicate in a cross cultural setting
• Ability to work independently
• Knowledge in developing policies, procedures and standards
• Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
• Travel within the region and provincially
