About The Position

This Social Worker position is located in City, State and will primarily cover the Emergency Department and float based on hospital needs. The role involves seeing 5-8 patients/consults per day and requires specific certifications including BLS, MSW, and 5150. The shifts are Wednesday to Saturday, from 16:00 to 02:30. The hospital has a 24/7 pharmacy and an in-house hospitalist or intensivist. Floor-specific orientation in the Emergency Department will be provided.

Requirements

  • Master's degree from a school of Social Work accredited by the Council of Social Work Education
  • Minimum of two (2) years of clinical and/or medical social work experience
  • Excellent customer service and presentation skills
  • Strong interpersonal and written communication skills
  • Ability to apply analytical and problem-solving skills
  • Proficient in managing multiple tasks or projects effectively
  • Detail-oriented with the ability to work independently
  • Ability to thrive in a fast-paced environment with changing priorities
  • BLS Certification
  • MSW
  • 5150 certification at the county level

Responsibilities

  • Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients.
  • Provide patient and family-centered care that respects personal self-determination.
  • Conduct comprehensive assessments and develop treatment plans addressing medical, social, and resource issues impacting patients and families.
  • Collaborate with healthcare team members to implement patient-centered treatment plans.
  • Engage in crisis intervention and high-risk screenings, as well as brief counseling and resource referrals.
  • Coordinate family and patient representative meetings with the healthcare team as needed.
  • Assist with post-acute placement for complex discharges, engaging appropriate agencies or community resources.
  • Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, and community resources.
  • Participate in hospital committees and contribute to the development and presentation of in-services for staff.
