About The Position

This Social Worker position is located in City, State and will primarily cover the Emergency Department and float based on hospital needs. The role involves seeing 5-8 patients/consults per day and requires specific certifications including BLS, MSW, and 5150. The shifts are Wednesday to Saturday, from 16:00 to 02:30. The hospital has a 24/7 pharmacy and an in-house hospitalist or intensivist. Floor-specific orientation in the Emergency Department will be provided.

Requirements

Master's degree from a school of Social Work accredited by the Council of Social Work Education
Minimum of two (2) years of clinical and/or medical social work experience
Excellent customer service and presentation skills
Strong interpersonal and written communication skills
Ability to apply analytical and problem-solving skills
Proficient in managing multiple tasks or projects effectively
Detail-oriented with the ability to work independently
Ability to thrive in a fast-paced environment with changing priorities
BLS Certification
MSW
5150 certification at the county level

Responsibilities

Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients.
Provide patient and family-centered care that respects personal self-determination.
Conduct comprehensive assessments and develop treatment plans addressing medical, social, and resource issues impacting patients and families.
Collaborate with healthcare team members to implement patient-centered treatment plans.
Engage in crisis intervention and high-risk screenings, as well as brief counseling and resource referrals.
Coordinate family and patient representative meetings with the healthcare team as needed.
Assist with post-acute placement for complex discharges, engaging appropriate agencies or community resources.
Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, and community resources.
Participate in hospital committees and contribute to the development and presentation of in-services for staff.
