About The Position

The Social Worker will provide casework, outreach, referral, and advocacy services to families. They will function as part of a medical and rehabilitation team to plan for patients' needs, requiring excellent clinical, oral, and written communication skills. The role involves creating written intake, case summary, and statistical reports. The Social Worker will understand and promote Elizabeth Seton Children’s Mission, Vision, and Core Values, participating in intake evaluations and assisting families with treatment plan follow-up. They will provide ongoing casework services, therapeutic intervention, and coordinate services with families and external agencies. Additionally, the Social Worker will assist with communication between family members/caregivers and medical staff, provide information and referrals to community support services, and carry a small caseload of Children’s Waiver cases. Support for the Social Work Manager in departmental functions, including transportation, is also required. Maintaining departmental policies, procedures, objectives, quality assurance, safety, environmental, and infection control standards is crucial. The role requires compliance with all CRC, Inc. policies and procedures, participation in in-services and staff development, and performing other assigned duties.

Requirements

  • MSW from an accredited University in Social Work
  • Licensed Master of Social Work Required
  • Minimum 1 year of experience in a pediatric clinical setting preferred.
  • Must meet or exceed all general health standards and requirements for the position as established by employer in compliance with NYSDOH and other regulatory agencies if appropriate
  • Must be able to stand and move freely while treating and supervising patients
  • Must be able to drive and travel throughout Westchester and Putnam Counties for Care at Home Program
  • Must be able to assist in the evacuation of patients as necessary
  • Must be able to lift/carry a minimum of 35 pounds

Responsibilities

  • Provide casework, outreach, referral, and advocacy services to families.

  • Coordinate the services of CRC, Inc. with families and appropriate involved agencies.
  • Assist with communication between family members/caregivers and medical staff during clinic appointments.
  • Provide information to clinic families and make appropriate referrals to community support services.
  • Carry a small caseload of Children’s Waiver cases in partnership with leading Health Home under supervision of SW Manager.
  • Support Social Work Manager in all activities pertaining to departmental functions, including Transportation.
  • Maintain departmental policies, procedures, objectives, quality assurance, safety, environmental and infection control.
  • Comply with established CRC, Inc. policies and procedures re: quality assurance, safety, environmental and infection control, confidentiality of patient information, customer service, attendance, punctuality, dress code, professional conduct, annual health assessments etc.
  • Participate in all departmental in-services and staff development activities.
  • Performs other duties as assigned.
