Your role is crucial in providing continuity of care for longer-stay patients, ensuring that their complex psychosocial needs are met throughout their hospital journey. You are a compassionate and patient professional, driven to support patients and families during challenging times. In this role, you’ll provide a comforting presence, actively listening, and offering tailored support that addresses psychosocial needs. For example, you might guide a family through the stress of a difficult diagnosis, offering resources and emotional support that helps them cope and adapt with clarity and understanding. Your expertise in crisis intervention and brief therapeutic interventions allows you to provide immediate, effective support while respecting professional boundaries and the limits of your role You are also a creative problem-solver, adept at multi-tasking and quickly assessing situations to implement appropriate interventions. This might involve addressing a patient's immediate needs during a crisis, such as helping a family find temporary support following an unexpected medical event. Your ability to think creatively ensures that patients receive the support they need in critical moments. Our team provides range of social work services including assessment, crisis intervention, counselling; bereavement counselling and comprehensive discharge planning with individuals and families.
