About The Position

The successful applicant will be part of a multi-disciplinary team responsible for the care of patients and families within their specific unit, reporting to the Clinical Services Manager, with professional accountability to the Professional Leader for Social Work. The social worker will be an integral part of the Social Work professional team, participating in yearly strategic planning initiatives and corporate professional development, and will support the unclaimed decedent process in the morgue for Medicine and Palliative Care patients as required.

Requirements

  • Ability to provide cross coverage within the medicine program
  • Masters degree in social work required
  • Three to five years experience working within an inpatient unit providing counselling for patients and families
  • Knowledge in community resources
  • Member of OCSWSSW and in good standing
  • Knowledge and experience working with relevant legislation (i.e. Substitute Decision Maker Act, Power of Attorney, consent and capacity, etc.)
  • Knowledge and experience in the psychosocial impact of chronic illness on the person and family
  • Excellent communication and interpersonal skills
  • Ability to work as part of a multi-disciplinary team
  • Maybe required to travel between and work at all Osler sites
  • Excellent organization skills and ability to set priorities as required
  • Good critical thinking and problem-solving skills required

  • Computer proficiency preferred

Responsibilities

  • Works collaboratively with internal medicine and palliative specialists, primary care physicians, nursing, allied health team and the patient/family to manage complex medical patients through psychosocial assessment, implementing a plan of care, monitoring and evaluating patient outcomes on an ongoing basis
  • Providing care in a compassionate and caring manner within a diverse cultural environment
  • Readily available for patient consultation requests from members of the CMC team
  • Participates in biweekly case management meetings by presenting on clinic patients including: plan of care, follow up and evaluation, using a chronic disease model
  • Providing health teaching to patients and families including thorough discharge instructions to ensure the continuity of care in the community
  • Maintains best practice through clinical research and literature review
  • Participates in program development and quality improvement initiatives
  • Understands the importance of building relationships between physicians, team members, patients and community partners
