A Social Worker with a Master's degree in Palliative Care Services is responsible for identifying and assessing various issues of patients/families seen that impact the overall health of the patient and serves as a liaison and resource in solving the issues. The candidate would serve as a liaison and resource to aid the patient/family in obtaining resources in the community. A working knowledge of services and resources within the community is required. The candidate must be able to work with the Palliative Care staff and outside agencies to formulate alternates to acute care when appropriate. The social worker will also work with the outpatient clinics to coordinate follow-up services and visits for patients within both the clinics and hospital as well as community resources. This position will also work with in-house patients and their families as needed. Flexibility to meet varying patient/family needs in the areas served. Mobility requiring frequent standing, walking and sitting. Must have interpersonal communication and problem assessment skills and be capable of independent judgment and action regarding psychosocial needs of patients. Adaptability to constantly changing work environment required. Excellent oral and written communication skills essential. Must be computer literate. Must possess sound decision-making abilities. A skill in priority setting and ability to deal with numerous issues simultaneously is a necessity. Ability to positively interact with people from all socio-economic and educational levels with tolerance and understanding.
