Requirements

Master's Degree in Social Work
Active, unrestricted licensure as one of the following: LSW (Licensed Social Worker), LMSW (Licensed Master Social Worker), LCSW (Licensed Clinical Social Worker), LISW (Licensed Independent Social Worker)
2+ years of experience in medical social work or case management
Ability to obtain and maintain multi-state licensure (minimum of 10 states – willing to obtain additional licenses) before completing new hire training (licensure costs paid by the company)

Responsibilities

Partner with Registered Nurse Case Managers to support members through assessment, care planning, and advocacy
Identify and coordinate community resources related to housing, transportation, financial assistance, safety, and behavioral health needs
Engage members telephonically and electronically to support positive health and lifestyle outcomes
Assist members in overcoming barriers to care and accessing appropriate services
Collaborate with interdisciplinary teams to address medical, psychosocial, and behavioral health needs
Support a diverse member population across all stages of life
