About The Position
United Hebrew Geriatric Center in New Rochelle is looking to add a Full-time Social Worker to our team of experienced professionals. As a Social Worker, you will provide essential services to residents and their families. Being rated as one of the top ten NY nursing homes by US News & World has allowed us to continue our mission, "We are kind people who treat you like family". United Hebrew is a beautiful long-term and rehabilitation facility in New Rochelle, Westchester County. We are rated by CMS as a 5-star facility, with 4 stars in staffing. Our high staffing levels allow nurses and CNAs to provide excellent and safe care to residents and patients. We put an emphasis on patient safety and staff work satisfaction. Requirements
- Bachelors or Masters degree
- Minimum of 1 year of Social Work experience
- Available to work from 9am-5pm weekdays
Nice To Haves
- License strongly preferred
- Experience in long-term care a plus
Responsibilities
- Assisting with admissions
- Assisting with discharges
- Scheduling care plan meetings
- Completing assessments
