About The Position

In this role, the Social Worker helps patients understand their rights and responsibilities and supports them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. Support may include crisis counseling, mental health services, and connections to community programs and resources. The position offers group supervision, education, training, and professional development opportunities.

Requirements

• Master's degree in Social Work (MSW).
• Licensed or registered in Indiana as required by state regulations (LSW or LCSW).
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint, and Outlook.
• Functional proficiency with DaVita-specific applications within 60 days.
• Dedication to caring for patients with chronic kidney failure.

Responsibilities

• Help patients understand their rights and responsibilities.
• Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
• Provide or connect patients with crisis counseling and mental health services.
• Connect patients with community programs and resources.
