About The Position

Georgia Hospice Care is seeking a hospice medical social worker to support patients and their families. The role provides psychosocial evaluations, ongoing counseling, bereavement services, and community education outreach. In accordance with each patient’s plan of care, the social worker provides physical, emotional, and spiritual support to patients and families. The position offers access to a supportive leadership team and professional development opportunities, with room for advancement.

Requirements

• A heart to serve patients and families and a passion for providing the best possible care.
• Master of Social Work degree from an accredited school of social work approved by the Council on Social Work Education.
• Current state license as a social worker.
• At least 2 years of clinical work experience; healthcare or hospice experience is preferred.
• Reliable transportation.
• Ability to sit, stand, bend, and move intermittently, and to lift at least 25 pounds and bear the weight of an average adult effectively.
