Mercy Hospital Ada, Oklahoma, is a 159-bed, acute-care facility serving Pontotoc County and the surrounding areas in south-central Oklahoma. Our hospital provides vital health care services to the Ada and surrounding communities.

Requirements

  • Bachelors required
  • Board Certified Social Worker, or related field (i.e., counseling) in the state of Oklahoma (CSW, LPCA, LCSW) within 6 months of hire
  • Handle with Care within 30 days of hire
  • Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.

Responsibilities

  • Assessing, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
  • Determines, priorities, provides and/or arranges for needed internal and external services/interventions.
  • Participate in case reviews to evaluate case management and progress.
  • Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Assists with discharge planning and processes.
  • Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Supervises and/or trains new staff, students, and interns.
  • Provides individual, family, and group therapy.
