About The Position
Provides social work services and discharge planning to patients and families in a healthcare setting to address individual health and psychosocial needs and promote wellness and cost-effective outcomes. The position also transitions patients to alternate levels of care and provides crisis intervention as needed.
Requirements
- Bachelor’s degree in social work from an accredited social work program.
- Current state social work licensure in Minnesota and North Dakota.
