About The Position

Suncrest At Brighton/Suncrest Hospice is a CHAP-certified hospice provider focused on changing the expectation of hospice care through exceptional patient care and investment in their people. They are seeking a compassionate Social Worker to provide psychosocial support to patients, families, referring sources, and team members navigating the challenges of serious illness and end-of-life care. The role involves advocating for patients and families, connecting them with resources, and collaborating with the interdisciplinary team to ensure their needs and preferences are met.

Requirements

• Master of Social Work degree that complies with CHAP and state of residence regulations.

Responsibilities

• Provide compassionate psychosocial support to patients, families, referring source and team members as they navigate the emotional, social, and practical challenges of serious illness and end-of-life care.
• Serve as an advocate, resource, and trusted member of the interdisciplinary hospice team.
• Assess patients’ and families’ psychosocial, emotional, spiritual, and socioeconomic needs and incorporate findings into the individualized plan of care.
• Counsel patients and families while providing emotional support, crisis intervention, and guidance as they navigate illness, loss, grief, and significant life transitions.
• Educate patients and families about available community resources and connect them with appropriate programs, services, and support.
• Advocate for patients and families to identify barriers to care and the hospice team to develop appropriate solutions to ensure their needs, preferences, values, and goals are heard and addressed.
• Participate in interdisciplinary team (IDT) meetings and collaborate with physicians, nurses, chaplains, aides, facility staff, and other members of the care team.
• Maintain accurate, thorough, and timely documentation in accordance with hospice standards and regulatory requirements.
